Atrial septal defect, or ASD, is a congenital heart defect in which blood flows abnormally between the two atria of the heart. Normally, the atria are separated by a wall called the interatrial septum. In ASD patients, this septum is defective allowing blood flow between the two chambers. During fetal development, when the interatrial septum is being formed, a small passageway called the “foramen ovale” is left open to allow the blood to bypass the non-functional fetal lungs while the fetus obtains oxygen from the mother’s placenta. At birth, as the lungs become functional, the changes in pressures between the two sides of the heart force the opening to close. However, in as many as one fourth of all adults, this opening does not close properly resulting in ASDs. The severity of a defect is measured by a net flow of blood, or a SHUNT. A shunt can be in either direction. Commonly, the condition starts as a “left-to-right-shunt” due to significantly higher blood pressure in the left side of the heart. This is because the left side has to pump blood all over the body while the right side only needs to send it to the lungs. Fortunately, in a majority of people the defect is relatively small; the shunt is negligible and does not cause any symptoms. When the defect is large, a clinically significant left-to-right blood flow may overload the right side of the heart, resulting in its enlargement and eventually right side heart failure. Without treatment, other complications may also occur. As the right ventricle continuously pumps more blood to the lungs, the entire pulmonary vasculature may be overloaded and pulmonary hypertension may result. Pulmonary hypertension, in turn, would force the right ventricle to generate even higher pressures to overcome the high pressure in the lungs. In some cases, this vicious cycle may cause the blood pressure in the right side of the heart to increase to a GREATER level than that of the left side. If this happens, the shunt will reverse its direction; a “right-to-left shunt” will result; the oxygen-poor blood will flow from the right atrium to the left atrium and will be sent to all tissues of the body. Fortunately, this complication, called the Eisenmenger syndrome, usually develops over many years and occurs only in a small percentage of people with large atrial septal defects. Small ASDs do not cause any symptoms and may not require treatment. In fact, many small defects close on their own during early childhood. Large atrial septal defects that cause clinically significant symptoms usually require surgical closure. As a general rule, the earlier in life the surgery is performed, the higher the success rate and the lesser complications presented.

Autism spectrum disorder (ASD) is a neurological and developmental disorder that begins early in childhood and lasts throughout a person's life. It affects how a person acts and interacts with others, communicates, and learns. It includes what used to be known as Asperger syndrome and pervasive developmental disorders. It is called a "spectrum" disorder because people with ASD can have a range of symptoms. People with ASD might have problems talking with you, or they might not look you in the eye when you talk to them. They may also have restricted interests and repetitive behaviors. They may spend a lot of time putting things in order, or they may say the same sentence again and again. They may often seem to be in their "own world." At well-child checkups, the health care provider should check your child's development. If there are signs of ASD, your child will have a comprehensive evaluation. It may include a team of specialists, doing various tests and evaluations to make a diagnosis. The causes of ASD are not known. Research suggests that both genes and environment play important roles. There is currently no one standard treatment for ASD. There are many ways to increase your child's ability to grow and learn new skills. Starting them early can lead to better results. My diagnosis of Autism Spectrum Disorder came late in life. This is not unusual for people my age who, because we were ‘higher functioning’, would have been previously diagnosed with Asperger’s syndrome. Asperger’s syndrome wasn’t a diagnosable condition until the 1990s. By then, I’d been through my teenage years: bullied at school, socially isolated, lonely, and suicidal. I didn’t understand the world, and the world didn’t understand me. In 2013, Asperger’s syndrome was removed from DSM-5 (the 5th Edition of the Diagnostic and Statistical Manual of Mental Disorders) and placed under the more general condition of Autism Spectrum Disorder. My diagnosis at the age of 42 was a revelation. My story highlights the difficulties I faced, but also shows that there is a way through. What’s next? A world where we not only accept neurodiversity but actively seek out those who see the world in a different way. Space instrument developer at The Open University. Background in Physics and Space Science with over 15 years’ experience in mass spectrometry. Currently working on the LUVMI-X lunar rover. Diagnosed with autism late in life, James passionately believes that any expertise is not despite their disorder but because of it. Enjoys sharing own experiences and helping others, yet terrified of public speaking! ASDs are the most common subtype of congenital heart disease. Normal Development Overview The right and left atria emerge from a single chamber which is septated between the fourth and sixth weeks of development. Atrial septation develops in such a way as to allow significant shunting of blood from the emerging right atrium to the emerging left atrium, a physiological priority during fetal life. However, following birth this shunt must be rapidly ablated; consequently, the inter-atrial septum develops in such a way as to provide a "flap-like" system that can be shut quickly following delivery. Septum Primum Development (First Wall) The first septum to divide the common atrial chamber grows, ridge-like, from the superior aspect of the atrium downward and eventually fuses with the Endocardial Cushions which defines the inferior aspect of the chamber. The hole defined by the advancing edge of the Septum Primum and the Endocardial Cushions is known as the first hole (Ostium Primum). As the Septum Primum fuses to the Endocardial Cushions (Destroying the Ostium Primum), a second hole is formed in the middle of the Septum Primum, known as the Ostium Secundum (Second Hole), which continues to allow right-to-left shunting of blood. Septum Secundum Development (Second Wall) A second wall, known as the Septum Secundum, begins to develop directly to the right of the maturing Septum Primum. As the Septum Secundum develops, a hole is formed in its center known as the foramen ovale which continues to allow right-to-left shunting of blood. Because the superior aspect of the Ostium Primum eventually regresses, the Ostium Primum begins to act as a flap against the foramen ovale and in doing so allows only unidirectional movement of blood from the right to the left atrium. Delivery During fetal life the right atrial pressure is slightly higher than the left atrial pressure, allowing blood to shunt from the right to the left atrium and thus bypass the developing fetal lung. Following delivery, the atrial pressures switch, causing the flap-like Ostium Primum to shut the foramen ovale, limit shunting of blood between the two atria, and thus promoting blood to enter the now mature fetal lungs. #atrialseptaldefect #usmle #pathology #cardiology #physiology

Learn more by clicking here 🤍 Tutorial On Early Signs of ASD: The American Academy of Pediatrics (AAP), Centers for Disease Control, National Institutes of Health, autism advocacy groups and researchers have all drawn attention to the importance of the early detection of autism spectrum disorders (ASD) to make access to early intervention possible. The scientific literature indicates that the average age of ASD diagnosis is 4 years, despite the fact that about half of children with this neurodevelopmental disorder may be detected by age 14 months. The AAP recommends that ASD-specific screeners be administered within pediatric practices beginning at age 18 months. However, these screeners are imperfect and often require parents to provide 'yes' or 'no' answers to questions about the presence of certain behaviors. Yet ASD-related behaviors often present inconsistently, and are intermixed with behaviors seen in typically-developing children. In addition, ASD-related behaviors often differ from typical development in quality, not just in quantity. To improve recognition of the early signs of ASD among pediatricians, parents, and early intervention providers, autism researcher Dr. Rebecca Landa (🤍 of Kennedy Krieger Institute ((🤍 has developed a free 9-minute video tutorial on ASD behavioral signs in one-year-olds. The tutorial consists of six video clips comparing toddlers who show no signs of ASD to toddlers who show early signs of ASD. Each video is presented with voice-over explaining how the specific behaviors exhibited by the child, as they occur on screen, are either indicative of ASD or typical child development. Dr. Rebecca Landa is the director of the Kennedy Krieger Institute's Center for Autism and Related Disorders. The videos and information presented within this tutorial were obtained through her research, funded by the National Institute of Mental Health, focused on early markers of autism spectrum disorders and early developmental characteristics of children with and without ASD. In this video, I discuss the neuroscience of autism along with potential factors and mechanisms involved in the development of autism. TRANSCRIPT: Autism, also known as autism spectrum disorder, is characterized by symptoms that include impairments in social communication and interaction and restricted and repetitive behaviors. Although the neuroscience of autism is still poorly understood, autism is considered to be a complex developmental disorder that involves atypical brain organization starting early in development. Individuals with autism often experience a period of unusually rapid brain growth in infancy and early childhood. This accelerated brain growth is linked to an atypical pattern of connectivity between brain regions. A number of studies report that alterations in brain circuitry involved with social interaction and attention can be detected well before the symptoms of autism begin to appear. At this point, however, it’s unclear how brain overgrowth and atypical connectivity might be linked to the occurrence of autism symptoms. Research suggests that the risk of autism is strongly influenced by genetics, yet studies consistently report that environmental factors also play a large role. Although a number of potential environmental factors have been identified, the risk factors for autism are far from definitive, and it remains unclear which factors are responsible for causing an increase in autism risk, and which are associated in a non-causal way. The risk factors that are most strongly linked to autism are associated with the prenatal or perinatal period. Thus, it’s possible they might be responsible for disruptions to typical neural development, leading to symptoms of autism months or years later. How these risk factors might interfere with neural development is still uncertain, but hypotheses have suggested potential mechanisms such as epigenetic effects, inflammation, oxidative stress, or damage caused by oxygen deficiency. More work needs to be done, however, to fully elucidate the genetic and environmental risk factors for autism, as well as the mechanisms for the development of autism symptoms. ASD atriumlar arasında delik olmasıdır. ASD geniş olduğunda kalbimiz normalden daha fazla çalışır. ASD doğuştan gelen bir hastalıktır, bazen tek başına bazen de diğer kalp hastalıkları ile birliktedir. Bazı ASD hastalarında genetik geçiş çok güçlüdür ve ailede birden fazla hasta vardır. ASD belirtileri nedir? ASD bebek ve çocuklarda genellikle belirtiye neden olmaz. Ancak, ASD belirti vermese bile rutin çocuk doktoru muayenesinde kalp muayenesi sırasında ekstradan bir ses üfürüm duyulur. ASD’nin belirti verip vermemesi: ASD’nin genişliğine; kalp içindeki yerleşimine; başka bir kalp hastalığı olup olmamasına bağlıdır. ASD çok nadiren ve çok geniş olduğunda bebek ve çocuklarda belirti verebilir. Belirtiler kalp yetersizliğinden kaynaklanır. Bu belirtiler: bebeklerde emmeme; iyi beslenmeme; hızlı nefes alma; kilo alamama; büyümenin yavaşlaması ve sık solunum yolu enfeksiyonları geçirilmesidir. Fark edilmemiş ve tedavi edilmemiş orta ve geniş boyuttaki ASD’lerde belirtiler genellikle 40 yaş civarında çıkar. Bu belirtilerden bir tanesi kalbin hızlı atması yani taşikardidir. Kalbimiz atın dörtnala koşması gibi hızlı atar. Diğer belirtiler ise cildin mavi bir renk alması, nefes darlığı ve erken yorulmadır. ASD nasıl tedavi edilir? Küçük ASD’ler genellikle 2 ila 5 yaş civarında kendiliğinden kapanır. Zaten kendiliğinden kapanması bile bu küçük deliklerin tedavi edilmesi önerilmez. Bu küçük ASD’ler hem bir belirti vermez hem de kalbe ek bir yük getirmezler. Orta ve büyük genişlikteki ASD’lerin kendiliğinden kapanma ihtimali oldukça azdır. Belirti vermeyen ASD’lerde en az 2 yaşına kadar beklenir. Çünkü düşük bir ihtimal olsa da bu delikler kendiliğinden küçülebilir, kapanabilir. Belirti veren ASD’ler ise ameliyatla ya da kateterle kapatılır. Günümüzde ASD’ler en sık olarak kateter yöntemi ile kapatılır. Kateterle kapatma yönteminde cihaz kasıktan kalbe taşınır. Kateter kapama yönteminin ameliyata en önemli üstünlüğü kalbin durdurulmadan işlemin yapılmasıdır. Diğer avantajları ise göğsün açılmaması, yara izi bırakılmaması ve iyileşmenin kısa sürmesidir. ASD’nin çok geniş olması durumunda ya da kateterle kapamaya uygun olmaması durumunda ameliyat kaçınılmazdır. ASD ameliyatında delik yama dikilerek kapatılır. 【父母的擔憂:ASD孩子的行為、情緒及學習問題不斷】 【Parents' Concern – The Every Behavior and Learning Problems of my Child with ASD 】 在開展5天的 #ABA訓練 前,陽陽媽媽跟我們分享了她對陽陽的發展的擔心。 Before the 5 Day training, we had an interview with Frank’s mother in sharing her concerns of Frank’s development. 陽陽現在5歲6個月大,在香港就讀於一間特殊學校。他在2歲7個月大時確診患有 #自閉症。陽陽的媽媽反映陽陽有很多 #行為問題,不論是在家或學校裡,他都難以保持專注。他的學習、等待和自控能力都相對較弱,每天都會哭鬧和發脾氣,尤其是當他的要求被拒絕的時候。 Frank is currently 5 years and 6 month old, now attending in a special school in Hong Kong. He was diagnosed with ASD at 2 years and 7 months old. His mum reported that Frank presents many behaviour problems. He is unable to sustain attention nor focus on tasks at school or at home. Frank also has poor waiting and self-control skills; he will throw tantrum daily, especially when his requests are denied. 她亦發現陽陽有語言發展遲緩的現象,只能夠說簡短的句子。他在說長句子和執行指令方面有困難。 Frank also exhibits language delay, can only say short simple sentences. He has difficulty in speaking in longer sentences and following daily instructions. 媽媽希望在這5天ABA訓練中,陽陽在多方面的技能都能有所改善,包括 #聽從指令,#耐心等待,#獨立完成任務 和活動。 Mum hopes that in this 5-day training, Frank can improve various skills including following #instructions, #waiting patiently, and completing tasks and activities #independently. 下週,我們會跟大家分享陽陽在香港AP中心開始 #ABA訓練 前的情況及他在 5天內取得的顯著進步,敬請關注!
